Sunrise Health Service has been operating since 2003 and is the second largest Aboriginal Community Controlled Health Organisation in the Northern Territory.

Based in Katherine (only 3 hours drive from Darwin), Sunrise Health Service services 9 remote communities to the east of Katherine. We serve a population base, predominantly Aboriginal of approximately 3,800 people - many with chronic illnesses requiring early detection and amelioration.

Primary Health Care Manager

The Primary Health Care Manager coordinates and supports the overall multidisciplinary and integrated approach to the remote health team’s goals to manage illness, promote well-being, maintain health systems, provide education and training and encourage community health action. Additionally, the successful applicant will be responsible for managing all aspects of the day to day operations of the Health Centre including planning, portfolio management, pharmacy, and consumables as well as assets management. Building the capacity of Aboriginal Health Practitioners (AHPs) and other Indigenous staff to provide culturally safe health care is pivotal to this role. This position collaborates extensively with other areas and organisations, both internal and external, to achieve the best possible health outcomes. The Manager also participates in the emergency call-roster and supports staff in meeting after-hours needs.

Health Promotion Team Leader

The Health Promotion program is designed to build capacity within our communities to provide healthy living messages to youth and young families within our communities. The workers will also support activities of the Population Health Outreach Team under the direction of the Health Promotion Coordinator. The program includes all areas of health but has an emphasis on mental health, sexual health and healthy relationships and rheumatic heart disease prevention and treatment. The Health Promotion Team Leader role works to the direction of the Health Promotion Coordinator and provides supervision and support to the local Health Promotion Community Workers who provide these health awareness and support programs in our communities. The Team Leader also facilitates the Health Promotion team to support other programs in the Population Health Outreach program.

Intensive Family Support Manager

The Intensive Family Support Service is a program that is targeted to reduce child neglect with focus on improving parental skills and how to provide better care for children. Referrals come from 3 areas - from the Northern Territory (NT) Department of Children and Families (DCF), as well as from the community and self-referrals. The principle responsibilities of the position include the provision of Intensive Family Support Service (IFSS) assessment service interventions and counselling-related issues. These may include home-based and community based intensive service. Sunrise is reviewing the current program model in conjunction with Australian Childhood Foundation to make it more focused on community supports.

Dietitian

The Dietitian is part of a multidisciplinary Population Health team. The dietitian is responsible for the provision of public health nutrition and dietetic services and will provide one on one consultations and contribute to the planning, implementation and evaluation of programs that will improve nutritional status for people living in the Sunrise Health Service Aboriginal Corporation (SHS) region.

Primary Health Care Nurse

The Primary Health Care Nurse contributes to a multidisciplinary and integrated approach to the remote health team’s goals to manage illness, promote well-being, maintain health systems, provide education and training and encourage community health action. Building the capacity of Aboriginal Health Practitioners (AHPs) and other Indigenous staff to provide culturally safe health care is pivotal to this role. The position collaborates extensively with other areas and organisations, both internal and external, to achieve the best possible health outcomes. Other aspects of the role include managing a specific health portfolio and providing after hours emergency support services on a rostered basis.

Clinical Educator

The Primary Health Care Clinical Educator assists all Sunrise clinicians, principally Aboriginal Health Practitioners and Primary Health Care nurses, to develop their knowledge and skills to maximise their performance in a multidisciplinary primary health care work place. The position helps support an integrated approach to managing illness, promote well-being, maintain health systems, provide education and training and encourage community health action. The position collaborates extensively with other areas and organisations, both internal and external, to facilitate staff skill development, strengthen local clinical governance and support continuous improvement.

Health Promotion Community Based Workers

(Multiple positions/flexible hours)

Under the guidance of the Health Promotion Coordinator and the Health Promotion Team Leader, The Community Health Promotion Worker (CHPW) will promote healthy lifestyle behaviours within the community as part of the Health Promotion Team.

CHPWs are based in the community and will be actively involved in the health programs that relate directly to the community. They will work closely with the Health Centre Managers and the Population Health Team.

This position will also work with local community members, and other key stakeholders to develop resources and run community based activities that support and promote healthy lifestyle behaviours.

This role will require ongoing community consultation with clients and community members to ensure programs are meeting SHS priorities and community expectations.

Health Promotion Coordinator

The role involves working with community members, relevant stakeholders and Sunrise Health Service Aboriginal Corporation (SHS) staff to plan, implement and evaluate health promotion initiatives throughout SHS region. The role also involves devising a health promotion strategic plan around general health promotion and provides supervision of the Health Promotion Team including a Team Leader and up to 6 community-based workers.
